IMPORTANT SCALES

I may ask you to complete these scales during our treatment together

ADHD

Vanderbilt scales are used for diagnosis and for tracking the efficacy of the medication. There are four sets of forms:

Parent Initial to be filled out prior to medication 

Teacher Initial to be filled out prior to medication 

Parent follow up to be filled out during the titration process

Teacher follow up to be filled out during the titration process

ADHD

This scale helps identify and track symptoms of ADHD for young adults and adults.

MOOD DISORDERS

This is a list of symptoms that may indicate someone has bipolar disorder.

MOOD DISORDERS

This is a scale that helps identify if someone is likely to have bipolar disorder.

MOOD DISORDERS

This is a questionnaire which may indicate you are experiencing hypomania or manic symptoms.

DEPRESSION

This is a questionnaire which is aimed at quantifying depressive symptoms in children and adolescents.

DEPRESSION

The PHQ-9 is a depression symptom screener for adults.

OCD

This scale helps quantify the severity of obsessive and compulsive symptoms. Pages 1-4 are for you to fill out.

OCD

This assessment helps delineate the level of insight into obsessions and compulsions. This is a clinician administered scale.

Anxiety

This is a screener for anxiety disorders.
